Appointments:

Appointments can only be made via telephone or email ( strictly no walk ins )
Prior to attending your appointment you will be sent a link, via email, to complete your medical history form and Covid questionaire
On arrival please telephone 01332 331688 to inform us that you are outside the practice.
The provided hand sanitiser in our entrance MUST be used.
Please minimise personal belongings - only bring essential items to your appointment.
Currently there will be NO use of our toilet facilities.

Entrance to the building:

On admittance you will be taken directly to the surgery.
Staff will be using correct PPE at all times.
All payments for treatment will be contactless card payments.
Please make use of hand sanitiser on your exit.

Finally. if you have any of the following symptons please DO NOT attend your appointment:
New , continuous cough
New fever / high temeperature
New loss of, or change in , sense of smell or taste
